Medications at school
If a student needs over the counter or prescription medications during school, the ADMINISTRATION OF MEDICATION form must be returned to the school nurse prior administration. If you give permission to give your child Tylenol, Ibuprofen, or Tums at school but will not be providing your own bottle of medication, please be sure you fill out the Yearly Health Form on Skyward.
Immunizations
Please see the state requirements for school here. If you choose to sign a waiver for any of the required immunizations, please fill out and sign the waiver portion of this form and email to thompsonb@shelllake.k12.wi.us or print it out and return to the school nurse. WAIVER
Immunization Update
On May 20, 2024, the Wisconsin Department of Health Services released an update to the immunization requirements for children attending child care centers and schools in the State of Wisconsin for the 2024/2025 school year and following.
Special Dietary Restrictions
If your student has special dietary restrictions, please have your physician complete the form below and return it to the District Office, ATTENTION: Nurse Thompson & Josh Schmidt Special Dietary Restrictions Form
NorthLakes Community Dental Clinic will visit Shell Lake School 3 times a year to provide the following services:
- Oral health education and dental supplies
- Oral health assessment by a license dental professional
- Fluoride varnish application
- Dental sealants and sealant replacement where needed
